A Co-Decision-Making Agreement allows a person (the appointer) to choose a trusted person (the co-decision-maker) to help them:
Understand relevant information
Consider options
Express their decisions
The co-decision-maker does not decide on their behalf, but shares the decision-making. The agreement can cover personal welfare, healthcare, property, or financial affairs.
As part of their capacity assessment, the medical practitioner or healthcare professional will assess whether the relevant person has the capacity to enter into, vary, or revoke the co-decision-making agreement.
Assessment duration: 1 hour +
